Lasting Lifestyles in Monmouth County
Lasting Lifestyles offers free web-based support for Monmouth County residents and their families.
The Monmouth County Division of Aging, Disabilities and Veterans Interment announces the new free web-based support program for Monmouth County residents and their families entitled " Lasting Lifestyles." This program is intended to comprehensively assist persons with chronic conditions or disabilities and their informal caregivers. Through the newly enhanced Lasting Lifestyles program, they are able to provide tools, information, education and access to local professionals necessary for self-management of care and independent living.
The design and features of this wonderful support program empowers people and professionals to be proactive in their wellness planning and outreach efforts to make a difference. It provides the opportunity to be informed and connected to family and friends, no matter where they may live and work, as well as a professional network of resource and services for self-management of care and independence. Lasting Lifestyles serves as a cutting-edge model of health and wellness, supporting and addressing the needs of the "whole person."
